Medical matters and official complaints cannot be dealt with via this form. If you have a query regarding a medical matter please telephone reception to make an appointment to see the appropriate person.

We make every effort to give the best service possible to everyone who attends our practice.

However, we are aware that things can go wrong resulting in a patient feeling that they have a genuine cause for complaint. If this is so, we would wish for the matter to be settled as quickly, and as amicably, as possible.

To pursue a complaint please complete this form and the Practice Manager will deal with your concerns appropriately or, telephone the Practice Manager on 01449 740254 and you will be sent our Complaints/Compliments leaflet. You can, if you wish, put your complaint in writing to us, addressing it for the attention of the Practice Manager, or use the on-line Complaints Form. 

Our aim is to deal with your complaint in a timely fashion.  If, however, you are not satisfied with our response or the time it has taken for us to reply, you may also complain to the Parliamentary Health Service Ombudsman:
